Registered Medical Practitioner’s prescription for Mobility Aid
Medical Leave Certificates

Payslips for the last 3 months before accident (Including month of accident)
Inpatient Discharge Summary indicating date of admission and date of discharge

Death Certificate, Autopsy report, Coroner's findings
Proof of relationship between the Insured and Claimant (E.g. Copy of Marriage Certificate / Birth Certificate)
